Consultation appointments missed without canceling at least 24 hours in advance will incur the full fee. If you are interested in mental health treatment that may be covered under your health insurance plan, please complete the request for treatment form. Your primary address must be in Ohio to use insurance benefits with this provider. If scheduling a telehealth session, you must be in Ohio during the appointment due to licensing requirements.
